On-Line Enrollment Agreement

This online Access Agreement contains the terms that govern your use of the online services of First State Bank. You may access certain eligible accounts through the Internet.

Each banking product you have with us is considered an eligible account. They include, but may not be limited to, deposit accounts and loans that you maintain with us.

By signing this agreement, you authorize First State Bank to provide access to some or all eligible accounts. You may cancel your online access to your accounts by sending a secure message through NetTeller's Message Center; writing us at 706 Washington St, Mendota, IL 61342; faxing us at 815-538-2327; or phoning us at 815-538-2265.

Your accounts will be available at www.firststatebank.biz, (click on the My Account link at top of page) to obtain balances, transaction history, and other information. You must have a NetTeller ID and NetTeller PIN provided to you by the First State Bank and the required hardware and software to access your accounts online. In general, you will be able to access your accounts 7 days a week, 24 hours a day. Transactions entered after 5:30 PM on the site will not be posted until the next business day and bills paid after noon on Friday will not be processed until 2 AM the next business day.

At certain times, the Web site may not be available due to system maintenance or circumstances beyond our control. At those times, you should call, visit one of our bank branches, use an automated teller machine or use our voice response system (815-538-7466).

Business Accounts

If you are a business, any authorized representative of your business is permitted to use the online service if they are noted by you as a designated user of your account. We may, however, require authorized signers listed on the resolution to the agreement and abide by all of its terms and conditions.

You may also allow "view only" access for any of your employees. If you choose to provide viewing access to your account to another person, you are solely responsible for causing that person to use information for the purpose for which you have provided access. You are also responsible for causing that person to maintain the confidentiality of his or her password or login identification.

Electronic Mail (EMail)

We may communicate with you through email or through NetTeller's secure message feature. You must provide us or with a valid email or text message address. We may respond to your request by email, but we will never send you an email to ask for confidential information about your account(s). Furthermore, you cannot use email to conduct transactions on your accounts. For security purposes, email must not contain any confidential information. Please use NetTeller's secure message feature if your message contains nonpublic customer information.

Message Center

A secure message center for all online banking customers has been provided. This is a secure way for our customers to send us sensitive information. Once you submit the message, it is sent to a "message board". An email is then sent to the bank to let our personnel know that there is a secure message waiting for us. A bank employee then logs into the message center, retrieves the message and replies to the message. Once we have replied to the message, an email goes out to the customer letting them know that a secure message is waiting for them. The customer will log into their account and retrieve the message. At no time does the body of the message appear in an email.

First State Bank encourages our customers to use this secure message center when relaying sensitive information to the bank.

Bill-Pay Service

You may use our bill-payment service to pay bills. You can arrange to have bills paid at your option or a recurring basis. There is no limit to the number of payments that may be authorized.

For bill payments from your checking account, we are not obligated to make the payment unless your account has sufficient funds to cover the payment. If your account has overdraft protection and your bill pay requires use of this service you will be assessed an overdraft fee of $30 for each item paid. If you cancel bill payment, all pending payments associated with the account will also be cancelled.

Electronic Fund Transfer Provisions for Consumers

Electronic Funds Transfers that debit or credit a consumer's deposit account are subject to the Electronic Funds Transfer Act as implemented by Regulation E (12 CFR 205).

Liability for Unauthorized Electronic Funds Transfers (EFT)

Your liability for an unauthorized EFT or a series of related unauthorized EFTs will be determined as follows:

  • If you notify us within two business days after learning of the loss or theft of your password, your liability will not exceed the lesser of $50 or the amount of unauthorized EFTs that occur before notice to us.

  • If you fail to notify us within two business days after learning of the loss or theft of your password, your liability will not exceed the lesser of $500 or the sum of:


    • Fifty dollars or the amount of unauthorized EFTs that occur within the two business days, whichever is less; and


    • The amount of unauthorized EFTs that occur after the close of two business days and before notice to us, provided we establish that these EFTs would not have occurred had you notified us within that two-day period.


  • You must report an unauthorized EFT that appears on a periodic statement within 60 days of our transmittal of the statement to avoid liability for subsequent transfers. If you fail to do so, your liability will not exceed the amount of the unauthorized EFTs that occur after the close of the 60 days and before notice to us, and that we establish would not have occurred had you notified us within the 60-day period. You may, as applicable, also be liable for the amounts described in the immediately preceding sections 1 or 2 above.


  • If your delay in notifying us was due to extenuating circumstances, we will extend the times specified in the immediately preceding paragraphs 1, 2, or 3 above to a reasonable period.

  • You may notify us in person, by phone, or by email during a secure session using the appropriate form or in writing.

Online Statements

Online statements are offered for eligible deposit accounts, allowing you to replace your mailed (paper) statement with an electronic version (PDF) that you can view, save to your computer, or print at your convenience. At the bank's discretion and with your consent, any notices that normally are mailed to you may be delivered to you electronically. An online statement will replace your mailed statement in its entirety.

Consumer and business checking, savings, money market, and other deposit accounts that have recurring periodic statements are eligible to electronically receive statements online.

A periodic statement of your account(s) will be available to you via our Web site by clicking View. You will need to ensure your computer software meets the requirements provided on the Web site in order to view, print, and/or save your statement. You will be able to view the current month's statement as well as two previous statements.

You may cancel your online statements at any time. You may do so by:

    1. Logging into your account
    2. Clicking on the E-Statements tab
    3. Click on Documents and Settings
    4. Deselect the account you wish to stop getting e-Statements for; or


You may cancel your online access to your accounts by sending a secure message via NetTeller by clicking "Contact", writing us at 706 Washington St, Mendota, IL 61342, faxing us at 815-538-2327, or phoning us at 815-538-2265.

When you turn off this feature, you will begin to receive paper statements with your next statement cycle.

Fees

First State Bank does not charge for their internet banking product at this time. However, we reserve the right to assess a charge in the future.

Security

You will be given a NetTeller ID and a temporary NetTeller PIN that will grant you access to your NetTeller. After accepting the required disclosure, the system will guide you through the Multi Factor Authentication process. NetTeller will also prompt you to change your PIN the first time you access NetTeller. We recommend that you regularly change your PIN by clicking on the NetTeller "Options" tab.

Information exchanged through our online banking service is protected by advanced encryption techniques. Nevertheless, these security measures still require your responsible behavior in protecting your NetTeller ID and NetTeller PIN.

Privacy

We are committed to protecting your privacy. All information gathered from you in connection with online banking will be governed by our privacy policy. You can view this policy by clicking on the Privacy Statement tab in NetTeller.

User Responsibility

You are responsible for transactions conducted using a NetTeller ID and NetTeller PIN. You are also responsible for keeping this information confidential and for ensuring that you have logged out of the online banking service when your session is finished.

You agree that you will be the only user of your NetTeller ID and NetTeller PIN; that you will not transfer or disclose any of this information to any other person; and that you will be responsible for all usage and any fees associated with the use of other services accessed through online banking.

You agree to immediately contact us at 815-538-2265 of any actual or suspected unauthorized use of your NetTeller ID and NetTeller PIN.

You agree not to use online banking to conduct any business or activity or solicit the performance of any activity, which is prohibited by law. You also agree to comply with all applicable laws, rules, and regulations in connection with online banking. You certify that you are at least 16 years of age.

Except as otherwise required by law, rule, or regulation, we may change the terms of this agreement from time to time and at any time. When changes are made, we will update this agreement on our Web site. This agreement will be in effect from the date of your enrollment for our online banking service and during all times that you use NetTeller. Either you or First State Bank may terminate this agreement and/or your access to our online banking service at any time without notice.

If you do not access your accounts online for a consecutive six-month period, we may terminate your online banking service. If you wish to cancel your online banking service, send us cancellation instructions in writing to:
